Trust officers, individual trustees and professional fiduciaries are often the first outside professionals to see that a beneficiary is in trouble. Blackburn Clinical Services is an outpatient therapy and psychiatry practice in West Los Angeles. This page explains how a fiduciary can help a beneficiary start care, what information can flow back and how fees work. To discuss a beneficiary, call (888) 870 7405 or email info@blackburnclinical.com.

Signs a beneficiary may need clinical care
- Requests for distributions that are urgent, unexplained or escalating
- Missed meetings, unreturned calls, or a sudden change in how they communicate
- Signs of intoxication, or family reports of drinking or drug use
- Withdrawal from school, work or family, or long periods with no structure
- Statements of hopelessness, or any talk of self-harm (if there is immediate danger, call 911; the 988 Suicide and Crisis Lifeline is available by call or text)
How a referral works
- Talk with the beneficiary or the family first. The beneficiary, or a parent for a minor, decides to start care. A fiduciary can share our information and offer to help with logistics.
- Call or email us, or use our referral form. We can talk in general terms about whether our services fit, without the beneficiary’s identifying information.
- We contact the beneficiary to schedule an assessment. Care starts with a thorough assessment and a plan built around the person.
- Coordination, with consent. With the beneficiary’s written permission, we can coordinate with the people they choose. Without it, we cannot confirm or share anything.
Incentive provisions and treatment
Some trusts tie distributions to milestones such as school, work or sobriety. That is a legal and fiduciary matter for the trustee and the trust’s counsel. Clinically, care tends to go best when the beneficiary experiences it as help rather than as a test, so our role is clinical care for the beneficiary, and with the beneficiary’s written permission, we can share attendance and general progress with a trustee, nothing more detailed.
When the distribution or inheritance itself is part of the strain, a beneficiary can see therapy for sudden wealth and inheritance. Families who also share a company can see therapy for family business families.
Trustees and family members preparing to raise care with a beneficiary can use our guide on how to talk with a young adult about starting therapy.
Fees and payment
We are an out-of-network practice. We do not take or bill insurance, and we provide superbills for reimbursable services. Whether and how a trust pays for care is the trustee’s decision under the trust document. See Fees and Insurance.
Families of beneficiaries can read Helping a Young Adult Trust Beneficiary Start Mental Health Care. For a beneficiary who mainly needs structure and follow-through rather than clinical care, Blackburn Mentoring, a sister organization, has a page for trust officers and trustees. See every professional page at for professionals.
