When is anxiety more than ordinary worry?

Everyone feels anxious before a hard conversation or a deadline. The National Institute of Mental Health explains that anxiety disorders involve more than occasional worry or fear: the anxiety does not go away, is felt in many situations, and can get worse over time (NIMH). It names generalized anxiety disorder, panic disorder, social anxiety disorder and phobia-related disorders among the main types.

In daily life that can look like lying awake running through every way tomorrow could go wrong, avoiding meetings or phone calls, sudden waves of panic in a car or a store, constant reassurance-seeking, or a tight chest and stomach that never fully settle. Many people also notice they have started using alcohol or cannabis to take the edge off. If that is part of your picture, read about virtual dual diagnosis care.

How does anxiety therapy work by video?

Sessions are one-on-one with a licensed therapist, from a private room on a computer, tablet or phone, while you are located in California. The first step is an assessment of what the anxiety looks like, how long it has been going on, what you have already tried, and what else is happening in your life. From there your therapist chooses approaches that fit you, such as:

  • CBT (cognitive behavioral therapy): spotting anxious thinking patterns, testing them, and gradually facing avoided situations in planned steps.
  • ACT (acceptance and commitment therapy): making room for uncomfortable feelings while acting on what matters to you.
  • DBT skills: practical tools for intense emotion, distress tolerance and interpersonal effectiveness.

Our page on CBT, DBT and ACT explains the differences, and NIMH describes these and other evidence-based talk therapies on its psychotherapies page. Between sessions, most people practice skills in the situations that actually trigger their anxiety, which video makes easy to plan around: your therapist sees the room you work in and the life you are practicing in.

Is video a good fit for anxiety?

For many people, yes. Starting from home can lower the barrier for someone whose anxiety makes driving, traffic or waiting rooms hard. It also fits people whose schedules are already stretched. Video is not a way to avoid everything, though. If avoidance is part of the problem, your therapist will plan real-world practice with you, and some people later choose to come to our Los Angeles office when that becomes a goal.

What does the Concierge Outpatient Program add?

The Concierge Outpatient Program is an individualized alternative to IOP. Clients typically take part in 3 to 5 hours of focused one-on-one work each week, with no groups and no hour requirements. By video, it can combine individual therapy, family sessions, psychoeducation on emotional regulation and relationships, and coaching and case management. Hours go up or down as your needs change. Details are on the virtual outpatient program page.

What about medication for anxiety?

Some people with anxiety benefit from medication alongside therapy. Psychiatric care can be arranged and coordinated with your therapist; we explain how when you call.

When should you get help sooner or at a higher level?

  • If you have new chest pain, fainting or shortness of breath, see a medical provider. Panic can feel like a medical emergency, and only a medical evaluation can rule one out.
  • If anxiety comes with thoughts of suicide, call or text 988 now.
  • If you are drinking or using substances heavily to cope and stopping could bring withdrawal, talk with a medical provider before you stop.
  • If you can no longer leave home, eat or sleep, tell us on the first call; we will talk honestly about whether outpatient care is the right level.

Who do we see for anxiety by video?

We see adults and young adults across California, including professionals, college and graduate students, and people in Northern California who want care from a team that also offers more intensive one-on-one support. See virtual outpatient care for professionals and online therapy for Northern California. In Los Angeles, our anxiety treatment page covers in-person care. For teens 13 to 17, see online teen therapy in California.

How does payment work?

We do not take or bill insurance. As an out-of-network practice, we provide superbills for reimbursable services. See fees and insurance and the private pay therapy guide.
