Therapy and Concierge Outpatient Care by Video for Northern California

Blackburn Clinical Services offers therapy and the Concierge Outpatient Program, a one-on-one alternative to IOP with no groups, by secure video for clients located in California, including anywhere in Northern California. Our only office is in Los Angeles; Bay Area, Sacramento, Tahoe and North State clients work with us online. Call (888) 870 7405.

Monday to Friday, 9 AM to 5 PM. Secure video for clients located in Northern California.

What can Northern California clients do with Blackburn by video?

Everything on this list happens on secure video while you are located in California:

  • Individual therapy: one-on-one sessions with approaches such as CBT, DBT and ACT, chosen for you after an assessment.
  • Concierge Outpatient Program: typically 3 to 5 hours a week of focused one-on-one care, with no groups and no fixed hour requirement. By video it brings together therapy, psychoeducation on regulation, relationships and relapse prevention, plus coaching and case management.
  • Career coaching: strengths-based coaching by video, alone or with therapy.
  • Teens 13 to 17: see online teen therapy for California families.

Two things work differently. Psychiatric care can be arranged, and we explain how on the first call. Brainspotting, TMS and testing happen only at our Los Angeles office, so they are not part of video care.

How is the Concierge Outpatient Program different from a virtual IOP?

Most virtual programs that show up in a search are group IOPs. Ours is not an IOP. It is an individualized alternative built for people who need more than a weekly session but cannot, or do not want to, spend most of their week in groups.

The 12 to 20 hour figure is the range our existing program page gives for traditional IOPs. For the full comparison, read our virtual outpatient program for California, and for weekly care see telehealth therapy across California.

Who in Northern California usually calls us?

  • Professionals with no room for a group schedule: people in tech, finance, law, medicine, government and hospitality whose weeks are already full. Our professionals page explains how we fit around work.
  • People whose drinking or drug use and mood are tangled together. Our dual diagnosis approach treats both at the same time.
  • People stepping down from residential treatment who still need structure once they are home in the Bay Area, Sacramento or the North State.
  • People who tried group IOP and did not get lasting change, or did not want to share a group with neighbors or coworkers.
  • College students and their parents, from quarter-system campuses in Davis and Santa Cruz to semester schedules in Berkeley, Chico and Fresno. We can complete leave of absence and return evaluations when clinically appropriate.
  • Rural and mountain residents for whom the nearest private outpatient program is hours away.

Which Northern California areas have their own page?

Each page below covers local context, the county’s public behavioral health access line from the county’s own website, and how video care works there. We do not have an office in any of these places; care is by secure video.

Numbers come from each county’s or city’s official site, checked October 30, 2026; hours and eligibility vary, so confirm on the county page. The California Department of Health Care Services also keeps a statewide list of county access lines for substance use services. County systems mainly serve Medi-Cal members and people without insurance; private care with us can sit alongside them.

How big is the area we serve by video?

Large. As of January 1, 2026, San Jose had about 980,434 residents, San Francisco about 845,658, Fresno about 560,948 and Sacramento about 539,765, according to the California Department of Finance E-1 estimates. Add the suburbs, college towns, wine country, the coast, the Sierra and the North State, and many Northern Californians live a long drive from a private one-on-one outpatient program. Video closes that gap without asking anyone to move or take leave from work.

Does it matter where you are during a session?

Yes. You must be physically located in California at the time of each video session. Your home address alone does not settle it. That matters most for:

  • Tahoe residents who work or stay on the Nevada side in Stateline or Incline Village.
  • North State residents who travel into Oregon or Nevada.
  • Business travelers and anyone splitting the year with a home in another state.

If you will be out of state, tell us and we move the session.

What does a video session actually look like?

You join a secure video link from a private room on a phone, tablet or computer. The first sessions are an assessment, then you and your clinician agree on a plan. In the program, the week is a mix of individual therapy and other one-on-one components; in weekly therapy, it is one regular hour. Psychotherapy is a well-studied treatment for anxiety, depression, trauma and substance use; the National Institute of Mental Health describes the main types. Sessions run Monday to Friday, 9 AM to 5 PM, with some Saturday therapy appointments.

When is video outpatient care not enough?

Outpatient care, even several hours a week, is not right for everyone. If you are thinking about suicide, cannot stay safe, are having withdrawal symptoms, or cannot stop using despite serious harm, you may need a medically supervised or residential setting first. We will tell you honestly if that is the case and help you think through next steps. If you are in danger now, call or text 988 or call 911.

How does paying for care work?

Blackburn is private pay. We do not take or bill insurance, and we provide superbills for all reimbursable services so you can submit them for possible out-of-network reimbursement. See fees and insurance and the private pay therapy FAQ. We discuss fees by phone.
