How does San Francisco life shape what people bring to therapy?
San Francisco is a city and a county at once, with about 845,658 residents as of January 1, 2026, according to the California Department of Finance E-1 estimates. It packs that many people into roughly seven by seven miles, and the density shows up in session. Many clients share walls, share flats with roommates, or live alone in a studio where work and rest happen at the same desk.
The work culture here is intense in a particular way. Tech, finance, law, biotech and startups all run on long hours, equity that may or may not pay off, and layoffs that arrive by email. Hybrid schedules mean some weeks you are on BART or Muni every day and some weeks you barely leave the Mission or the Sunset. People tell us the city makes it easy to drink after work, easy to stay online until midnight, and hard to admit that things are not fine.
Common reasons San Franciscans call us: anxiety that has started to run the workday, depression after a layoff or breakup, drinking that went from social to daily, and the strain of being far from family in an expensive place. When mood and substance use are tangled together, our dual diagnosis approach treats both at once.
What public mental health help does San Francisco offer?
The city runs a 24/7 behavioral health access line at 888-246-3333 for information on mental health and substance use services, listed on the Behavioral Health Access Center page on SF.gov. The center itself is at 1380 Howard Street at 10th and takes drop-ins seven days a week; check the page for current hours. Public services and private therapy can run side by side, and we are glad to help you think through which fits.
Is weekly therapy enough, or do you need the Concierge Outpatient Program?
Most San Francisco clients start with weekly therapy. The program is for weeks when one hour is clearly not enough but leaving work, or sitting in a group of strangers, is not an option either.
| Your situation in SF | Usually a fit |
|---|---|
| Stress and sleep slipping, still functioning at work | Weekly therapy |
| Drinking or pills creeping up after hours | Concierge Outpatient Program |
| Back home after residential treatment elsewhere | Concierge Outpatient Program as a step down |
| Tried a group IOP, did not stick | Concierge Outpatient Program, one-on-one |
| Thoughts of suicide or unsafe use right now | 988 or 911 first, then a higher level of care |
We will tell you honestly if you need more than we offer, such as residential or a medically supervised setting.
Where can you take a private video session in San Francisco?
Privacy is the practical problem in a dense city. Clients use a bedroom with a white noise machine at the door, a parked car, a bookable phone room at the office, or a walk in Golden Gate Park with earbuds when the conversation suits it. Fog and hills can drop a cell signal, so test the spot once before your first session. You must be physically in California during each session; if a work trip takes you out of state, we reschedule.
How does paying for care work?
We are private pay and do not bill insurance. We give you superbills for reimbursable services, which you can submit to your plan for possible out-of-network reimbursement. Details are on our fees and insurance page and the private pay FAQ. Fees are discussed by phone.
